Introducing MonitorIM in Primary Healthcare

The IHS Project and the Main Family Medicine Centre (MFMC) in Prishtina held a coordination and information meeting on 12 August to advance the implementation of MonitorIM. This is a digital self-management tool supporting patients living with diabetes and hypertension.

The meeting focused on the application’s key elements, past experience from its practical use among patients, and the implementation plan for 2026. Participants were also introduced to the latest upgrades of the MonitorIM application.

Discussions addressed the roles and responsibilities of family doctors, nurses, endocrinologists and other health professionals in patient enrolment, education, monitoring, and follow-up, with a focus on integrating the programme into routine patient care.

Participants also discussed coordination between the MFMCs in Prishtina, Peja and Gjakova and the General Hospitals in Peja and Gjakova, as well as specialized tertiary care institutions where applicable, exploring practical modalities for collaboration and stronger engagement of primary healthcare settings in implementation.

The meeting concluded with agreement on the next implementation steps for MonitorIM, contributing to continued efforts to strengthen patient engagement and self-management of diabetes and hypertension.
